Living with diabetes can feel overwhelming at times, but understanding it
can help you feel more confident in managing your health.
Join our Chronic Care team for a supportive, practical session about
making sense of your diabetes numbers and learning what you can do
with that information.
What we’ll talk about:
- Understanding your numbers: What blood sugar readings and A1C mean, and why they matter.
- Knowing what affects your blood sugar: How food, movement, medications, stress, and other factors can play a role.
- Everyday diabetes management: Practical steps you can take to care for yourself.
- Knowing when to ask for help: When to reach out to your Chronic Care team and what support is available.
